After an executive session, can Action Items be noted in the minutes?

The rules regarding the content of minutes for an executive session are the same as the usual rules regarding the content of the minutes. So yes, the main motions considered by the assembly should be recorded in the minutes.

Yes, in fact you must record all motions (action items) in the minutes. But since these are executive session minutes, they remain secret. And to make sure they remain so, they are approved/corrected in executive session too.

If you are a public body subject to Sunshine Laws, the rules will be somewhat modified. Check with your attorney.
