In a recent meeting, the chair had as the first order of business the election of an at large board member. Shouldn't the first order of business after calling the meeting to order be the approval of the agenda?

If the assembly intends to have an agenda (which would seem to be the case, since elections are not normally quite that early in the meeting), yes, adopting the agenda is generally the first order of the day (although there are some exceptions - mainly conventions).

 

By the way, the term order of business properly refers to the full sequence of business at a session, not to a single item in that sequence. (An agenda is a particular order of business adopted for a specific session.) A single item of business is known as an order of the day.
