A board is made up of 5 members. Is the presiding official, president or CEO considered as one of the board members thus being 6 members?  Or is it a 5 member board with a presiding official,president or CEO?  And would this presiding official, president, CEO be considered part of the quorum?

You'll need to check your bylaws to see whether the President or CEO is a member of the board. A board member certainly does not lose his status as a board member if he serves as presiding officer.

And would this presiding official, president, CEO be considered part of the quorum?

If he is a member, yes.
