No, it means that no individual member can force an item onto an agenda.  Presumably a member of the body that is meeting.

What sort of meeting is this?  If it's a board meeting then yes, only a board member can propose a change to the agenda, but it takes a vote of the board to make it so.   If it's a membership meeting, then the rule is similar, except board members have no role.

No, items can only be added by the assembly itself, by majority vote. No individual member can add items, regardless of who that member is.

It's not clear what assembly is meeting. If it is a meeting of the association as a whole, then any member can propose that an item be added, but it will be up to the association's membership, by vote, to determine whether to add the item. If it is a board meeting, then only a board member can propose to add the item, and it is up to the board, by vote, to determine whether to add the item.

Additionally, adopting the agenda as a whole, after any amendments are handled, also requires a majority vote for adoption.

I would note that our answers, of course, are based upon the rules in RONR. Any rules on this matter in your organization's rules or applicable law would take precedence.
