If bylaws provide a nominating committee must be elected by the membership, but they are simply appointed by the president, and the elections for officers occurred as scheduled, are the elections invalid as the nominating committee who proposed the slate had not been properly elected by the membership? In other words, is this a continuing breach that can be questioned after the fact?

Nope.

One would hope that nominations from the floor were accepted and write-in votes were permitted.

No. The breach ended when the committee made its final report and went out of business, so to speak.
