A subsidiary motion to Amend is voted on before the pending main motion. If the motion to Amend is adopted, the main motion as amended is voted on. If the motion to Amend is rejected, the unamended form of the main motion is voted on. See RONR (10th ed.), p. 125, ll. 14-19.

A motion to Lay on the Table requires a second, is neither debatable nor amendable, and requires a majority vote. RONR (10th ed.), §17, pp. 201ff.

A motion for the Previous Question requires a second, is neither debatable nor amendable, and requires a two-thirds vote. RONR (10th ed.), §16, pp. 189ff.

If a motion is is made and seconded, then an amendment is moved and seconded, which is voted on first?

The amendment.

Is it mandatory that if the amended motion passes, to still vote for the original motion?

The amended motion doesn't pass until it is voted on. The amended motion is voted on after the motion to amend the original motion is adopted.
