non profit social service org, meeting went into closed session, staff excused, discussion took place, staff asked to rejoin, motion made, vote taken

How do I record this in the minutes?

example of minutes:

A motion was made that the Board go into a closed session to discuss XXX.

(Accepted: XXX/XXX)

All staff excused from the meeting.

Closed session terminated.

A motion to XXX was made.

(Accepted: XXX/XXX)

Is this correct??

There should be two sets of minutes--one for the open, or public, session, and a separate set for the executive session. The form and contents of each set are the same, but the minutes of the executive session are approved in executive session.
