Voting without a motion

Generally, yes. There is potentially some latitude in this requirement, however, particularly in the case of a small board.

In any event, whether the question comes before the assembly by means of a member making a motion or the chair "assuming" a motion, the chair should be certain to "state the question" so that all members are clear on exactly what they are debating and voting on.

Yes.  Otherwise how do you know what question you are voting on?  If there is some way that the question is made clear before the vote, then that thing is a motion—or should be.
