Committee Chair appointment and replacement and Chairman of the Board position

If a Board of Directors elects the Committee Chairs who has the authority to relieve a person as Committee Chair? Does the President, Chairman of the Board or does the Board of Directors vote on this to resolve? Is it allowed that the President also hold the positoin of the Chairman of the Board?

Whichever body elected the Committee Chair also has the authority to remove/replace him (RONR pp. 293-299). It is often the case that the President will also be the Chairman of the Board although it is not required that the same person fill both offices.
