Who has to request a motion at a board meeting?

No one needs to request a motion, although the chair may sometimes ask if anyone wishes to make one.

If an agenda item has been approved by a committee and presented to the board, is a motion and a second required at the actual board meeting.

Business is brought before a board by a member of the board making a motion. In small boards (no more than about a dozen members present), seconds are not required.

Well, yes you need a motion, which is a proposal that the assembly take some specific action, so that everyone knows exactly what action the group is being asked to take  before they decide - via a vote - whether or not to take that action.

If this is a small board - meaning no more than about a dozen members present - seconds are not required. In larger assemblies, seconds are used to indicate to the chair that there is more than one member who wants to consider the motion. See RONR, 12th ed. 49:21 for a description of the small board rules.

A committee may make a recommendation to the board, assuming that it reports to the board. The recommendation is made in the form of a motion (rather than just an agenda item). The person reporting on behalf of the committee, usually the committee chair, is usually the one who makes the motion. Motions made on behalf of a committee do not require a second.
