writting a proposal for a Board policy

When a proposal for a Board policy is presented and passed as written, must it appear in the policy manual exactly as it was written?(not revised in any way)

Can anyone give me a reference from Robert's Rules for this?

Common sense says that the policy should be reproduced exactly as adopted. You don't need RONR to tell you that (and you can rest assured that no one else will be able to use RONR to suggest otherwise).

And for those who don't have common sense one option is to take a belt and suspenders approach and specify in the motion that the approved policy will be included in the policy manual word-for-word.
