motions - duly made and seconded... by who

When using the term "on motion duly made, seconded, and carried, the Charter was approved as amended", in what cases do you need to mention in the minutes WHO made the motion, and WHO seconded the motion? Is it necessary to mention the names of those who made and seconded the motion?

The name of the maker of the motion is included in the minutes, and the seconder is not included unless the assembly orders it. RONR (11th ed.), p. 470, ll. 26-28. RONR does not use the term or form you have typed. Look at RONR (11th ed.), pp. 472-473 to see how things should be worded in the minutes.
