"Addendum" to meeting minutes?

Is it appropriate to add an "addendum" to previous month's meeting minutes that details something that was intended for that month's meeting but did not take place due to lack of time? 

Example:  Staff member's annual goals were to be developed at the August meeting, but there was not time to get to this agenda item.  BOD President and staff met the day after the meeting to create a draft of goals that were then sent to Board members via email for review and input.

Should this information be added to the August minutes as an "Addendum" for an agenda item that happened outside of the meeting?  There was not time to table it til the next month's meeting.  Does it need to be recorded somewhere because it was on the agenda?

No, the information in question should not be included in the minutes. The minutes are a record of what was done at the meeting. The fact that this item was included on the agenda is immaterial.

No.  The minutes of the previous meeting should include only those things that were actually done at that meeting.

If there were items on the agenda that were not reached during the previous meeting, they can be brought up under Unfinished Business and General Orders.
