A bylaws amendment could be proposed any number of times, as long as the procedures for amending the bylaws are followed.

Although someone could bring it up again, you could cut off debate on it by adopting the Previous Question (as suggested in an earlier post). The Previous Question is a motion to close debate and requires a two-thirds vote.

So a rule to never bring something up again doesn't stop someone from bringing it up again at any meeting.

What is meant by "active"? Either you're a member or you're not. If there are 5 members, a majority would be 3.

Yes, if they are no longer members. A quorum is based on the number of members present.

The general membership could do whatever it wants with the committee's recommendation.
